RF Health Insurance Termination Report

Description

Use this report for employees whose health insurance is terminating.

Central Office runs this report and sends it to the campuses on a monthly basis.

The following table describes the required and optional parameters to use to run the report:


Parameter	Description (click in the field to see the List of Values)
Required Parameters
Start Date		First day of the preceding month.
End Date		Last date of preceding month.

The table that follows describe the report output:

Column Heading	Description of Column
Operating Location	Campus code and name.
Employee Name and Address	Employee name and address.
Employee Number	The unique identification number assigned to each employee in Oracle.
Benefit Carrier	Name of benefit carrier. For example: RF PPO
Assignment Status	Assignment status category. For example: Terminated Processes Pending
Coverage	Type of coverage. For example: Employee only
Coverage Date	Start date of coverage.
Termination Date	Benefit termination date.
DOB	Date of birth.

The report is available in PDF and ASCII (TEXT) format. PDF is a formatted, printable version. ASCII can be easily pulled into a spreadsheet or word processor.
Once you select a format and run the report, you cannot change the output format. You will need to run the report again and select the desired output format on the Submit Request window.
